Gradys Stroke and Trauma Centers are marquee services and both are on a growth curve. Each year approximately 500 stroke patients are discharged from Grady directly to home. Many more return home by way of acute or subacute rehabilitation. The majority of these patients lack follow up rehabilitation care. A large portion suffer from ICU-acquired weakness, cognitive dysfunction, PTSD, anxiety, and depression. Many do not understand the pathophysiology of their medical condition or grasp their secondary prevention strategy. If they have a family, many questions and concerns of the caregivers have no systematic process to address their concerns or to coordinate care.
