Patient Safety and QualityJob Summary
Under minimal supervision, collaborates with and institutes process improvements as indicated for patient safety across the organization. Collects and analyzes data and prepares reports for various patient safety, quality and regulatory initiatives impacting all of Denver Health's clinical programs. Facilitates project identified through occurrences, common cause analysis, root cause analysis, industry trends, or other improvement methods. Is an active participant in department meetings and develops strong relationships with key leaders (nursing, physician and operational) throughout the health system. Assists with audits, reviewing clinical guidelines, and ensuring up-to-date policies and procedures. Plays a key role in leading the hospital accreditation survey process, including participating in tracer activities, ensuring continual readiness, and supporting all site visits (announced and unannounced) related to Denver Health's clinical accreditation. Supports other quality initiatives in collaboration with additional members of the Patient Safety and Quality team and with other departments as necessary.
Essential Functions:
General Operations: ? Monitors and maintains Quality Improvement initiatives in a timely manner. Demonstrates a working knowledge of State and Federal Regulations as well as The Joint Commission standards, applying them to assigned projects. ? Using sound organizational skills and the ability to prioritize, independently manages work functions related to the position, including ability to define problems, collect data, establish facts and draw valid conclusions. ? Displays ability to apply professional experience to carry out complex tasks and assignments, ability to deal with multiple tasks, and ability to manage multiple assignments and varying priorities. ? Acts as a Facilitator / Educator / Clinical Quality Resource Person for the organization, including sharing up-to-date knowledge of regulatory standards, industry best-practices, quality improvement tools and methods, and relevant clinical guidelines and protocols. ? Understands policies and procedures related to clinical quality and patient safety. ? Audits and monitors documentation, data, and performance of care delivery to ensure compliance with quality and safety initiatives and regulatory standards. ? Verifies the validity of data prior to reporting to the Director. Submits reports to department in a timely manner and keeps leadership aware of any significant changes in quality performance. ? Participates in regulatory audits as requested. ? Managing of Occurrence reporting system. Monitors and provides direction and guidance, managing the occurrence reporting system (STARS) (50%)
Collaboration: ? Works in collaboration with clinical and ancillary departments as assigned to develop indicators and projects consistent with quality goals. ? Encourages open communication and increased reporting of safety, quality and regulatory events, by responding to occurrences in a constructive and non-punitive manner. ? Assists Patient Safety team in the development and implementation of strategies and tools to educate members/staff/practitioners and increase accountability for quality, safety and meet regulatory requirements. ? Meets with personnel from other departments to facilitate development of action plans in response to data identified in reports or trends identified from safety event reporting. (20%)
Professional Accountability: ? Disseminates educational materials and provides education to members and practitioners to help promote quality and safety initiatives and meet regulatory requirements. ? Participates in and, when appropriate, leads performance/quality improvement activities. ? Attends committee meetings relative to the Patient Safety and Quality department. ? Attends all department meetings and represents the Patient Safety Department in other assigned meetings. ? Identifies a minimum of three goals annually for professional growth and development. ? Seeks opportunities to upgrade role-related skills. ? Responsible for maintaining the confidentiality of all business/work. ? Assists in managing the flow of information between internal and external customers. ? Is a team player and can work independently. (20%)
Data Analysis: ? Collects, analyzes, and reports clinical indicators as appropriate. ? Collects and analyzes data from various sources with an eye for detail and accuracy. ? Reviews medical records accurately for specified indicators. ? Reviews reports to analyze information needed for specific projects. ? Identifies problems with documentation, compliance, and communication and provides appropriate feedback in a timely manner. ? Performs chart review as indicated or requested. (10%)
Education:
Bachelor's Degree Required
Work Experience:
7-9 years Minimum of seven years of professional nursing experience; prefer one or more years of experience in healthcare quality with a good working knowledge of Performance Improvement methodologies. Required and
7-9 years Experience in performing medical record review. Required
Licenses:
RN-Registered Nurse - DORA - Department of Regulatory Agencies Required
Knowledge, Skills and Abilities:
Computer proficiency in Microsoft Office
Knowledge of The Joint Commission and CMS Conditions of Participation standards preferred.
Skills to include collecting and compiling data for QI efforts, decision making skills regarding appropriateness of care review, ability to write clearly and concisely for various audiences.
Ability to be flexible and handle multiple tasks, simultaneously.
Ability to communicate in a professional and positive manner and to work independently and consistently meet schedules and deadlines.
Demonstrates constructive, accurate and effective communication skills and promotes effective communication between team members.
