Under general supervision, the Lead Licensed Professional Counselor (LLPC) provides integrated behavioral health as a keymember of the primary care team. The LLPC abides by guidelines and expectations specific to the clinic in which they providepatient care. The LLPC strives to create positive customer service experience by committing to the highest standards set forth byDenver Health. The LLPC provides as needed: case management and consultation, case coordination to facilitate referrals tospecialty care, and crisis intervention.
Essential Functions:
Direct patient carePerforms patient care in accordance with evidence-based clinical framework. Monitor clinical progression. Document care reflecting the patient's clinical progression per regulatory and state guidelines. Responds to patient and referral needs, maintains a safe environment of care, and ensures the maintenance of patient confidentiality. (40%)
Clinical OperationsProvides basic on-the-job training of assigned duties and assists the supervisor with the day-to-day activities, Caseload distribution, Supplemental clinical supervision and case consultation for assigned team members (25%)
EducationFacilitates both educational and therapeutic groups as well as individual sessions and fundamental case management. (10%)
Policy and PracticeDevelop & sustain standard of work. Development and tracking support of Gemba metrics (10%)
Staff ManagementAssists in overseeing the activities of the staff which includes but is not limited to sourcing, hiring, and training new team members, creating and managing schedules, evaluating performance, and providing coaching to engage and motivate team members, identifying and applying career advancement opportunities for team members, and helping to resolve employee issues and disputes. (10%)
For Patient Care Positions:* Ensures all duties, responsibilities and competencies are conducted in a manner that is effective and appropriate to patients/clients to whom care/service is being provided.* Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient's status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided.* Employee has completed and met their clinical competency standards.* Supports monitoring of compliance with Federal, State and Accreditation bodies (Random audits of clinical documentation). (5%)
Education:
Master's Degree Required
Work Experience:
4-6 years Clinical experience post master's degree Required and
1-3 years experience working in an in-patient treatment setting. Required and
1-3 years working in an in-patient treatment setting. Required
Licenses:
LCSW-Licensed Clinical Social Worker - LCSW State Licensing Board Required and
LPC-Licensed Professional Counselor - LPC State Licensing Board Required
Knowledge, Skills and Abilities:
Knowledge and the ability to audit for compliance of State, Federal and Agency mandated documentation.
Critical thinking - the identification of the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
Computers and Technology, Microsoft Office skills required.
Knowledge of electronic health record preferred.
