Patient Care Programs Coordinator - Skull Base Coord
JOB SUMMARY:
Coordinates and supports individual patients and families throughout point of care program(s) to ensure program goals are effectively executed with some guidance. Assures coordination of diagnostic evaluations, treatments, surgeries and follow-up appointments throughout the course of care/episode for a specific patient population.
Your Role:
The Skull Base, Patient Care Programs Coordinator will be a joint position between Otolaryngology, Head and Neck Surgery, and Neurosciences. You will be responsible for seamless patient flow from the referral through post-operative care and after. Daily tasks include:
-Corresponding with patients and referring providers to intake new referrals
-Contacting patients with skull base disease and their families via phone and e-mail to schedule coordinated appointments
-Ensuring that appropriate documentation is received and uploaded to the medical record
-Documenting correspondence and daily tasks to ensure that no patient is lost to follow-up
-Providing regular reports to O-HNS and Neurosciences team regarding current status of referrals and appointment scheduling
-Interfacing with medical professionals to ensure that care is coordinated and communicated clearly to patients
-Providing a high level of customer service and empathy toward patients and families
-Tracking and presenting program data, metrics, and volumes
-Providing insight into program improvements and working with strategic marketing to grow our Skull Base volume

KEY RESPONSIBILITIES:
Partners with clinical care staff and providers to ensure all regulatory requirements are met to maintain patient care needs, program guidelines and/or required accreditation standards.
Acts as a point of access for patients, referring providers and the Vanderbilt community.
Develops both short and long term goals to ensure programs' success.
Provides support to patient care team to market programs and streamline business models to allow for appropriate growth.
Establishes contact with existing, newly diagnosed, potential, or referred patients by facilitating access to providers, educational materials and communication with care providers.
