Part Time Community Educator - Nutrition Services Diabetes Prevention Program

The Nutrition Services group at ChristianaCare is looking for Community Educator to join its team, to coordinate and deliver public awareness and education activities for Nutrition Services Diabetes Prevention Program.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Develops and delivers community education and training programs for the Christiana Care Employees and the community
Establishes and maintains professional relationships with department leaders, community leaders, employees, and program participants
Organizes and participates in events, promoting Preventive Medicine programs including the diabetes prevention program
Maintains professional competence by participating in local and national training sessions and conferences
Partners with registered dietitians to facilitate diabetes prevention program classes including frequent participant follow up and coaching, outcome collection, and requirements related to CDC recognition
Participates in regular program and department meetings
Attends outreach events and activities as needed
Completes required reporting in a timely matter
Will be required to become a Certified Twistle builder. Helps multiple departments build Twistle projects for program enhancement and evaluation. Maintains this certification annually.
Curriculum development for the Weight Loss programs that are used by both the Weight Management and Nutrition Services teams.
Additional patient monitoring and data collection for the Weight Management Obesity Medicine Program.
Markets DPP and MDPP programs as well as Nutrition Services programs to Primary Care offices by going to health fairs, presenting at population health meetings, may need to do some weekend marketing at sporting events.
Working on expanding coverage for the DPP program to include additional insurance payers.
Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
This is a Part Time (32hrs per week), Monday to Friday with 2 late evenings up till 6:30pm.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's degree in Nutrition, Public Health, Social Work, or related field required
One year teaching/training experience required.
Public speaking, group facilitation and excellent writing skills required.
Health Coaching certification preferred
Computer experience required.
Demonstrated ability to work with diverse populations required.
Bilingual language skills are a plus.
