ChristianaCare is currently seeking a Director of Quality and Performance Improvement for our exciting new Hospital at Home program. The Director is accountable for driving successful quality outcomes and performance for the Hospital at Home initiative. The Director will be responsible for weekly volume reports to CMS, reporting of federally required measures, ensures follow-up and closure on all ChristianaCare R2L reports, and works collaboratively with key system leadership developing and implementing workflows and processes to reduce patient harm (CLABSI, CAUTI, Readmission rates).

Directs and manages all quality and performance improvement functions for Hospital at Home.
Ensures compliance with all CMS Conditions of Participation, Joint Commission standards, state, federal, and private payer statutes and regulations.
In collaboration with the healthcare analytics team, creates user-friendly data dashboards for timely awareness of current performance, trends, and opportunities for improvement.
Collaborates with key health system leaders to develop and implement workflow and processes focused on managing key performance indicators (KPI's), and preventable patient harm (CLABSI, CAUTI, Readmissions).
Using industry standard performance improvement methodologies, develops and implements strategies for pursuing opportunities identified by data analytics, audits, and other methods.
Prepare reports and presentations for the Center for Virtual Health Quality Committee, and other health system committees as necessary.
Serves as quality & performance improvement expert lead for Hospital at Home operations.
Collaborates with the project management team and data analytics team to assess performance to contract requirements and utilizes data to drive operational improvements. Creates action plans as necessary to address contract, regulatory and/or accreditation requirements.
Works independently on projects/reports as directed by Corporate Director of VP of the Center for Virtual Health (CVH).
Master's Degree in Business Administration, Hospital Administration, Health Care Administration or related field within 2 1/2 years of hire.
Two to three years' experience in quality and/or performance improvement in healthcare related setting
An equivalent combination of experience and education may be substituted.
Nurse or other clinical candidates must have an active, Delaware professional license
Certification in Healthcare Quality (CPHQ) is preferred, candidate must obtain within 1 year if not achieved at time of hire.
