The incumbent is designated as a Primary Care (PC) Provider assigned to the East Orange Primary Care Clinic within New Jersey and under the direction of VA New Jersey Health Care System. The primary purpose of the Provider is to provide comprehensive medical care to a panel of patients within the assigned location. GENERAL DUTIES: Performs an advanced, comprehensive, and holistic health assessment, including a relevant health history and physical assessment. Adapts assessment tools and techniques according to individual client needs and cultural context Determines the need for, orders, performs, receives, and interprets the appropriate diagnostic tests. Analyzes and synthesizes data from multiple sources to establish a differential and working diagnosis. Analyzes and synthesizes data from multiple sources to identify and monitor a health condition. Applies knowledge of pharmacology including pharmacokinetics, pharmacodynamics, and best practices in selecting, prescribing, and monitoring drugs to treat conditions, diseases, disorders, and injuries within the scope of practice of outpatient primary care. Communicates verbally and in writing history and physical assessment findings, diagnosis, and treatment plans. Communicates progress and treatment options with the client and other members of the team when indicated. Consults with other health care providers in an appropriate and timely manner. Critically appraises and applies current, relevant research into clinical practice from an evidence-based framework. Collaborates with the client and health care team in management and monitoring of a health situation by: Assessing client needs by compiling qualitative and quantitative information about clients (e.g. epidemiological information, interviews, surveys, research findings, and community assessments) Participating in the development of health promotion/prevention programs based on assessed needs, culture, evidence-based strategies, and available resources for clients who are healthy or have acute or chronic conditions. Participating in health promotion/prevention programs in partnership with others including the community, colleagues, and other sectors. Applying principles of teaching and learning when providing health education to individuals, families, and groups. Monitoring, evaluating, and modifying health promotion/prevention programs in partnership with the client and other members of the health care team. Encouraging client to take responsibility for maintaining and/or improving health by increasing knowledge of, control over, and influence on health determinants. Implements primary, secondary, and tertiary prevention strategies for individuals, families, and communities or for specific age and cultural groups, in accordance with population health principles. Accepts sole responsibility and accountability for all actions taken within his or her scope of practice. Incorporates professional, legal, and ethical decision-making guidelines into the practice of Medicine. Demonstrates leadership in clinical practice to meet multiple health care needs. Participates in interdisciplinary and/or inter-professional quality improvement and other meetings as related to her/his practice. Participates in emergency preparedness and environmental health planning as part of a team. Accepts personal responsibility for ongoing professional development related his/her competence, including, but not limited to, continuing education related to prescriptive authority and health management. Work Schedule: Full Time, Monday - Friday; 8:00AM - 4:30PM Financial Disclosure Report: Not required This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. If selected for this position, you must meet specific individual eligibility requirements, and submit your application within four months of appointment.
