Role Summary:
The IRF PAI Coordinator assumes the task to complete each IRF-PAI form completely, accurately, and timely. Maintain a high level of professional expertise and serve as a resource to the Rehabilitation unit and to Vanderbilt Wilson County Hospital.
KEY RESPONSIBILITIES:
- Complete and transmit the IRF-PAI to CMS consistent with regulatory guidelines. Collaborate with Intake Coordinator and admitting rehabilitation physician to determine anticipated case mix group (CMG) to be assigned to each patient admitted.
- Collect patient demographic, clinical and quality measure data and enters on the IRF-PAI.
- Communicate with coders within 4 days of admission to determine etiologic diagnosis and comorbid conditions present on admission to be included on the IRF-PAI.
- Communicate estimated length of stay to case manager for discharge planning and discussion at team conference.
- Participate in team conferences and review documentation concurrently throughout patients' stay to ensure accuracy and compliance.
- Educates and trains staff, including physician, nurses and therapists, on documenting to ensure plan of care and supporting documentation reflects the need for intensity and medical necessity of services provided,
- Tracks and reports regularly on 60% rule compliance.

- Demonstrate and promote positive relations with all staff and co-workers in a friendly manner.
- Provide instruction/training on Quality Measures Scoring for rehabilitation staff and other hospital employees when necessary.
- Coordinate Quality Measures competency testing for designated rehabilitation staff.
- Communicate with the Program Director when unable to resolve any data or timeliness issues related to the IRF-PAI.
- Print the IRF-PAI in a timely manner and send - to Heath Information Management for inclusion in the medical record
- Ensure the IRF-PAI is entered into the appropriate medical record.
- Assist as required by the Program Director with inputting other patient data into the appropriate computer programs.
- Participate in quality improvement activities by identifying performance improvement opportunities in collaboration with rehabilitation leadership.
- Tracks and reports regularly on outcome data generated through IRF-PAI submissions.
- Attend hospital committee meetings as assigned by the Program Director.
- Maintain current knowledge of PPS regulations related to participation requirements for inpatient rehabilitation facilities.
- Act as a resource to the Inpatient Rehabilitation Program and the hospital.
- Attend Inpatient Rehabilitation Program meetings and provide education as requested
- Demonstrate and promote positive relations with all staff and co-workers.
- Assist in the assessment of patients referred for Inpatient Rehabilitation by gathering pertinent information, communicating with the Medical Director, initiating the admission process when appropriate and communication with all interested parties.
- All other duties as assigned.
