The Health Navigator I Scott & White Health Plan (SWHP) is a professional or non-professional with special training whose role is dedicated and tailored to meet the unique needs of the most vulnerable members of their communities, the dual eligible Special Needs Program (D-SNP) beneficiaries who are largely elderly, and poor many of whom are fragile and vulnerable. They have worked or have experience in health care or social services sectors. The essential duties of the Health Navigator I SWHP include the following:
Create connections between vulnerable populations and health care systems.
Manage care and care transitions for vulnerable populations.
Ensure cultural competence among health care professionals serving vulnerable populations.
Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, optimal functioning and management of activities of daily living and nutrition.
Advocate for assigned underserved beneficiaries to receive appropriate health care and community services.
Build capacity for each beneficiary to maximize their health, behavioral health and every day functional capacity.
Outreaches (telephonically and in person through potential home visits as needed to the Special Needs population) to each of their assigned lower risk beneficiaries.
Provide and coordinate services such as a Health Risk Assessment.
Member education, advocacy and coordination of beneficiaries care are essential services the member will receive from their navigator.
Responsible for a panel of approximately 200 beneficiaries to arrange and coordinate services for and ascertain that each and every assigned beneficiary has had a HRA completed within 90 days of enrollment.
Assess and coordinate with other team members that the beneficiary is making progress towards the goals as stated in the individual care plan.
The ideal Health Navigator I SWHP candidate will have good communication skills, demonstrate cultural competence, and have working knowledge about the health care system and community services.
