You Will Be Responsible For:
Â·Coordinates care of patients at risk for readmission from admission through 30 days post-discharge.
Â·Executes readmission prevention strategies.
Â·Enables and manages readmission prevention measures at all levels of care (hospital andcommunity, patient and provider).
Â·Reviews and analyses the readmission prevention process.
Â·Arranges for post-discharge support services if needed.
Â·Coordinates post-discharge phone calls.
Â·Provides direct interaction with care providers such as direct care nurses, physicians, social support and utilization (social services, care management), post-acute care providers (home health, LTC, rehab, medical home).
Â·Remains flexible to changing systems, is expected to demonstrated quality and effectiveness in work habits and treats staff, physicians, patients and families with consideration and respect.
Â·Recommends new services that can better serve our patients in regards to preventing readmissions.
Â·Provides community awareness programs to educate residents and referring agencies of services offered.
Â·Effectively communicates with all levels of the hospital and medical staff.
Â·Provides, coordinates and directs the activities of various levels of assigned nurses and ancillary personnel in the provision of patient care utilizing both professional and independent judgment.
Â·Participates on various hospital committees and task forces as required.
Â·Collaborates with other hospital departments including Medical Management & Case Management to promote quality and safety for all patients.
Â·Understands the Computerized Physician Order entry process and proficiently uses the electronic medical record for retrieval of patient information.
Â·Monitors Electronic Medical Record for new physician orders and diagnostic results.
What you will need:
A registered nurse with BSN required
2-5 years nursing experience required
Demonstrates knowledge of community resources and medical community
Advanced practice degree preferred
Home health or public health nursing is helpful
Provides supervision and guidance to transitional nurses
