Provides the administrative, operational, quality, fiscal and human resource leadership for UVA transitional care services (UVA Transitional Care Hospital, off-site transitional care services, hospice and future transitional care development areas). Responsibilities include effective resource utilization; short- and long-range planning; and program and policy development, implementation and evaluation. As a member of the division’s leadership team, the individual requires a vision for the future of healthcare and expertise in continuous quality improvement, business development, customer service, resource and financial management, efficient and effective system processes and professional development. Works collaboratively with the medical directors of all entities, as well as the department chairs and division chiefs.
Essential Duties and Responsibilities:
1.Serves as the executive and operations leader for transitional care services. Integrates the philosophy, mission, values and goals of the Health System into the direction and operational plan for the service line.
2.Provides leadership and direction to the transitional care service line. Ensures availability of required resources for delivery of patient care services. Oversees and promotes the development of strong and collaborative clinical and operational partnerships among the UVA medical faculty, staff, advisory boards, SOM Clinical Departments, Medical Center departments and services (e.g., Marketing, Finance, Environment of Care, Therapies, HSCS, ancillaries, etc.), and other programs (Hospital Education Program) that interface or impact UVA transitional care services. Communicates the requirements associated with serving this population with all entities, and works collaboratively to promote and coordinate the provision of excellent care to patients and their families.
3.Works collaboratively with the leadership teams within the transitional care service line and Medical Center Human Resources to establish criteria for recruitment, selection, retention, performance assessment, promotion and competence for all direct reports and their subordinate staff. Works collaboratively with the Chief Nursing Officer and the Dean of the SON to assess, develop and implement strategies that enhance workforce development in nursing and other clinical disciplines.
4.Shares accountability with the UVA transitional care services leadership team and associated medical directors for the quality, safety, integration and efficiency of clinical services and performances within the Health System. Reviews, assesses and recommends policies and practices for appropriateness within transitional care settings.
5.Oversees the development of UVA transitional care services, goals, priorities and monitoring metrics. Actively promotes service excellence of the patient care experience within a patient-, family- and employee-focused environment. Actively participates to resolve all concerns and complaints and to achieve service excellence at the highest level.
6.In collaboration with the UVA transitional care services leadership team and the Chief Marketing Officer, oversees development and deployment of the image, marketing, public relations, community service and all other recognition of UVA Transitional Care Services.
7.Defines, develops and directs the implementation of goals and programs for patient and support services.
8.Ensures continual improvement in clinical care within a cost-effective environment in collaboration with the other leaders within the Health System. Identifies quality improvement initiatives and oversees implementation in order to improve patient care services. Fosters a multidisciplinary approach to quality improvement work with a focus on improved coordination of services and enhanced clinical outcomes. Ensures the patient care delivery systems include consideration of acuity, patient mix, community needs, the knowledge and skill of staff, available resources and established healthcare benchmarks.
9.Functions as an integral member of the division leadership team. Participates in the organization’s decision-making structures and processes in order to ensure delivery of appropriate services to patients. Identifies and articulates the common needs and viewpoints of nurses and patient care staff to appropriate decision-making groups, and communicates the implications of environmental changes at the patient care level to hospital leadership. Participates with key Health System leaders in developing, reviewing and ensuring the Health System’s mission, strategic and operational plans, budgets, resource allocation and policies are carried out.
10.In collaboration with the UVA transitional care services leadership team and Health System leadership, leads the UVA Transitional Care Service Line in developing clinical affiliations. Assists the UVA Transitional Care Service Line in being attractive to insurance carriers. Oversees and implements care and utilization management initiatives and strategies. Assists UVA Transitional Care Service Line in becoming a competitive provider by utilizing strategies to increase market share.
11.In collaboration with the UVA Transitional Care Service Line Leadership Team and medical directors, ensures, as possible, compliance with all applicable Joint Commission and other regulatory requirements that impact the service line.
12.Fosters effective relationships with the department chairs and division directors within the School of Medicine, with the foremost representatives of the clinical staff and with Health System leadership to establish a quality agenda that reaches throughout all levels of the organization.
•EDUCATION: Bachelor's degree required; Master's degree in Business, Healthcare Administration, Nursing or equivalent graduate degree must be obtained within 4 years of hire or transfer. A clinical background is preferred.
•EXPERIENCE: 10 years of experience in healthcare with 5 years of leadership, preferably operational experience in a long-term, acute care hospital and/or a large healthcare organization, demonstrating leadership and strategic thinking.
o Possesses well-developed written and oral skills.
o Must possess knowledge of laws, regulations and accreditation standards for hospitals.
o Possesses personal qualities of integrity, maturity and flexibility coupled with high energy, tenacity and resourcefulness.
o Demonstrates key accomplishments in the areas of employee engagement, patient satisfaction, financial performance, strategic operations and quality improvement.
o A clinical background is preferred.
