The Contra Costa County Health Services Department and the CCRMC (Contra Costa Regional Medical Center) Hospital & Clinics are offering excellent career opportunities for qualified individuals interested in the Medical Social Worker II positions located in Martinez, CA. The vacancies are located in the Public Health AIDS and STD Program, Contra Costa Health Plan Authorization Unit, George Miller Wellness Center and CCRMC Hospital & Clinics. The successful candidates will provide a full range of counseling and related social services to patients within the healthcare system of Contra Costa County. Incumbents will be responsible for conducting interviews with patients and relatives to obtain data regarding the patient’s physical, social and emotional history; identifying patients requiring special services and making appropriate referrals; developing and maintaining patient client care plans; conducting discharge planning activities; and assisting medical and other professional staff with in-depth group and individual counseling problems by leading supportive counseling programs. When assigned to a Team Leader role, incumbents will provide ongoing lead direction to other medical social workers.
The ideal candidate will have the following:
Ability to apply the principles and techniques of rehabilitation counseling
Ability to establishing rapport with clients and their families
Knowledge of psychological and social aspects of physical and emotional disturbances
Excellent interpersonal skills and the ability to work in a team environment
Ability to adapt easily to varied and changing priorities
Effective verbal and written communication skills
Minimum qualifications include a Master’s degree in Social Work, Psychology, Counseling or a closely related behavioral science field and 2 years of full-time experience, or its equivalent, providing counseling and/or social work services to medical patients.
Substitution for Education and Experience: Possession of a Bachelor’s degree with a major in Social Work, Psychology, Counseling or a closely related behavioral science field and either: a) 2 years of full-time experience as a Medical Social Worker I in Contra Costa County or b) 3 years of full-time experience providing counseling and/or social work services to medical patients.
Substitution for Academic Major: 1 additional year of qualifying experience of the type noted above may be substituted for the required major.
